A rope or leather harness can hold it in place, but I prefer inflatable plugs to adjust the size for situation. They come with removable inflation tubes for discreet wear.

Think you need a larger plug. They can be made custom for the size you need. The big problem is sizing what is needed.  I did like VeryBadWizards comment.
