You need to agree an obvious, quick action that the sub will give if they get into distress. If their hands are free, maybe they tap you rapidly 2 or 3 times. Possibly they can blink a couple of times or move their head in a particular way. Pick something that is distinctive and if in doubt stop and check in with the sub. It is definitely better to be safe than sorry when you’re indulging in such an extreme kink.
